Why doesn't a Ready for Sale app appear in the app store?

The app was uploaded to the app store on September 17, 2014.
On October 13, 2014 the app was rated as Ready for Sale but it still hasn't appeared in the store.
What could be the reasons?

You have auto-publishing turned off. Either when creating an application, or when uploading a new version, there is a fad - immediately publish after the approval, or wait for manual confirmation. So you probably chose the second one - look for the publish button in the store.

Either they did not publish it themselves, or distribution was chosen only in some countries.
What does the app store say when you try to visit the app page?

You need to check the list of countries of distribution and the start date of sales. If everything is ok with them, then wait a while, applications do not appear in the App Store immediately, it may take several hours.
