Why do some providers resolve the wrong IP?

Suddenly - users began to complain about a broken site. We quickly figured out that DNS was the culprit.
There are 40 zones - regular subdomains with A and CNAME records.
There are also records including:

* A base_ip
sub-domain A subdomain_ip

When mass pinging a sub-domain using services on the Internet, almost all servers respond with subdomain_ip, but somewhere around 5-10% respond with base_ip
ns-s (reg.ru) respond correctly. Prompt - where to dig?

It turned out to be Reg.Ru's fault. 1/30 times their servers gave the wrong IP.

When making changes to the DNS - do you change the version of the zone?
