Why are photos from the iPad camera not being added to upload to the site?

Laravel 5 web application. Ad site.
In your personal account, you can attach a photo to your ads. The usual form for uploading a file. On the computer, a disk overview opens to select a photo, on a mobile device (tablet or phone) a menu appears with the choice of Photos from the library or Photos from the camera. He selects ready-made photos from the media library, takes pictures from the camera and inserts them into the form.
When I want to add a photo from the iPad camera - a black screen. Camera doesn't show anything (iPad Air, iOS 9.1)
With iPhone 5s, iOS 9.1 is added. On Android gadgets is added.
I don't even know how to phrase the question correctly. Why doesn't my ipad camera work? Feature of the device or it may depend on the site?

UPD: I tried to work on the iPad through the xCode emulator to catch the error. It turns out that the emulator does not have a camera. Photos are loaded from the media library. Where else to dig?

The problem may be with the ipad and in the code.
Upload from iphone, or other ipad. Better yet, use the iOS Device Simulator that comes with xCode.
Look in the browser what happens when you download, what data you get, what it gives.
In any case, more information is needed, since this is fortune-telling on the coffee grounds.
