A couple of questions:
1) Is everything done from scratch?
2) Are there already some solutions for IP telephony or do you need to start everything from scratch? And if from scratch, is it a hardware solution or a cloud one?
3) In what city is the organization located?
4) Will it be necessary, in addition to the development, to create a telephone infrastructure in the clinic itself?

Are you sure that you need development from scratch? View ready-made solutions with an eye to refinement to your requirements. Surely you do not have unique processes that cannot be implemented in a ready-made solution.
