Which front + backend + database connection to choose for data entry and storage?

I'm going to deploy a database with a nice web interface through which I will enter data.
I would like a convenient solution with a minimum entry threshold.

mysql + phpmyadmin ;)
apache or ngixn
is available in ruby, but I'm not sure that it is included in the "nice web interface"

php - process the web form and send a request to the database html css - create the
database web form you want, for processing php there are three options
nginx + apache2
nginx + phpfpm
