What steps should be taken to move a heavily used PostgreSQL database to another server with minimal downtime?

Once again about backup/replication.
There is a database on the server 9.3 (windows), which is actively used, which does not allow it to be stopped for longer than 5 minutes. But on this hardware, there is a performance drawdown and it is required to transfer the database to another server 9.4 (linux).
What is the action algorithm?
PS I read everything I found, but I'm not sure of any of the methods, or I can't understand the algorithm of actions.

Replication, DNS name change.
You need to look at the versions, it may be necessary to raise replication by 9.3 and then upgrade to 9.4
