What services and technologies are best used to create a personal electronic medical record?

Idea:
I have accumulated a lot of disparate information in paper and electronic form. Periodically, there is a need to simply show the doctor a history of one of the types of diseases "on demand". I visit different clinics, often moved to different cities and regions. There is no question of digitizing X-ray images.
Required requirements:
0. The solution is free or by subscription. Best of all - the ability to deploy yourself.
1. Storage of scans (images), text, pdf documents, tags, dates
2. Export / import in csv / xml format
3. Convenient text search
4. Export to pdf for viewing on tablets
5. Platform-independent solution. Web/java
Requirements desirable:
6. Encryption of data and data streams.
7. Reports, data visualization. For example, a graph of when I had ARVI for 10 years, what painkillers I took without complications, what vaccinations I did.
8. Knowledge base. Wiki?
Usage scenarios:
1. Demonstration to the doctor at the reception. The patient came to a new/paid clinic, the doctor asked questions. Search and demonstration of information on the tablet
2. Data digitization. Scans are entered into the database with notes (date, tag).
3. Search the knowledge base for treatment by symptoms. This refers to the history of diseases and notes to it.
Solutions:
Offhand, you need a picasa / digiKam photo repository with a Wiki and views by categories.
