What's with the weird Smatry bug?

Hi all.
Installed phpTODO script ( phptodo.godshell.com )
Error:
Fatal error: Uncaught --> Smarty: Unable to load template file 'index.tpl' <-- thrown in /Applications/MAMP/htdocs/libs/sysplugins/smarty_internal_templatebase. php on line 127
Guys, what could be the problem? Never met there.

There may be another option, after installing phpTODO you pushed the php script into the index.tpl file, but you cannot write pure php in smatry.

The index.tpl file is lost somewhere, or the paths to it are not correctly spelled out
