What plugins are there for cross-browser tab/password synchronization?

From a working computer, I would like to be able to synchronize bookmarks / passwords to my home computer, and vice versa.
Working - Win, home - Mac.
Work - Chrome, home - Safari.
The offer to switch to one browser is not accepted :)
