What do you consider the most unsuccessful decisions in the field of designing a graphical user interface?

I am writing an article about the curvature of user interface designers. Article for personal purposes.
We need illustrations of real applications or a website in which action buttons have incomprehensible labels or inappropriate graphic icons.
You also need an application or site that has a toolbar consisting of graphic icons that do not know what they mean, and at the same time do not have signatures.
In general, cases where the user experiences severe difficulty in using the program or site.
Do you have such unfortunate samples in mind? The examples I have from the 2010 articles have already been fixed or these sites have died completely. But I do not believe in the triumph of reason - there are still a lot of crooked designers :)
PS Friends answered that the number of terrible decisions is decreasing. Now you can find uncomfortable ones or ones drawn in the old fashioned way (not responsive, menus of pictures instead of web fonts), but there are few frankly terrible ones.

For example: it seems unfortunate that on the Toaster, when on the question page, the menu item "All Questions" is active. Especially if you get to the question page from your feed.
