Visual Studio Express 2013 for Windows or Visual Studio 2013 Pro/Ultimate/Community updated from SP2 and above - the fact is that WP8.1 SDK is automatically installed with SP2, otherwise you need to manually install it. Well now there it seems only with SP4 all VS.
And for testing - either you need to install an emulator, or a real smartphone
