1) /blog/category/seo
/blog/kakie-adresa-sdelat
2) /blog/seo
/blog/kakie-adresa-sdelat.html
In the first case I will pull out from the category table by category, in the second case I will pull out entry by .html

Not essential. But I would do this:
/blog/seo/
/blog/seo/kakie-adresa-sdelat
