Web design is the field of graphic design of the site, layout is a way to "pull" the designer's layout onto the site. In any case, these two areas are connected with graphics and not with the programming of the "brains" of the site. I suppose a web designer is different from just a designer in that he understands the web interface, knows how to think in CSS framework blocks. Of course, a full-fledged web designer must understand how to implement a layout in reality, which means a priori to have the skills of a layout designer.
Bottom line - do not toil, just call yourself a web designer in the fullest understanding of this profession.
