As far as I understand, the algorithm should be as follows:
CPM is considered. Either you set it manually (if you are placing with paid impressions), or it is ctr*cost per click*1000. All ads that can be shown to this user are selected. The ad with the highest cpm is selected (well, or such an ad is selected with a high probability).
Those. The higher the CPM of an ad, the higher the chance of it being shown. If you pay for clicks, then an ad with ctr x and CPC y will be shown with the same probability as an ad with CTR (x/2) and CPC 2y . Well, vice versa. An ad with cpm x*y*1000 will have the same impression probability (if you pay for impressions).
