BSD license ? Allows you to use the code anywhere, but with attribution.

MIT License . Very similar to BSD and meets all your requirements (read the "comparison with other licenses" section at the link).

You can also look at the Apache License, in some cases it seems to me more suitable than BSD, it is most often used for Java libraries / projects.
