Something simple to create forms with entry in MS SQL Server?

Good day!
Given: a purely technical base, which works within 5 advanced users. Access is through SSMS, employees work with views. To display graphs/reports, Reporting Services is used, where a new report is created in 5-10 minutes. Everything suits everyone, the logic lives in sp and triggers, it takes zero point zero tenths of the time to support, with security and so on. everything is fine.
Required: There was still a need to make only one form with a slightly advanced logic - change of responsible, change of status. Just for the sake of this, making a WebForms application is a little lazy, there is nothing similar “a la Access forms” in the standard functionality.
From what I dug up, only InfoPath is similar to a first approximation, and even then it is not clear whether there will be more trouble here than with WebForms and whether it can live in any browser without hacks.
Maybe someone can suggest something? Thanks in advance.
