Server health monitoring software

Greetings!

Colleagues, tell me, maybe someone came across. You need software to monitor the current state of the server, it is desirable that there be a web interface so that the customer can independently control the operation of the server in real time. The site runs on FreeBSD.

It is necessary to control:

1. The state of nginx / Apache (working / not working, the number of active connections)
2. The availability of the site running under Apache
3. The state of PHP / MySQL
4. CPU load / Memory consumption (what processes)

It is desirable to have an alert system , in case of the fall of any vital services
