I'm trying to send notifications with filters, but notifications are not filtered. I used the date of the last entry into the game - last_access_range=2016.01.01-2016.03.01, notifications are sent to all players of the application without filters (judging by the counter)

Indeed, recently there have been some alterations on alerts.
The redesign of alerts naturally affected the delivery schedules of alerts, as the new alerts work in a different way than the old ones. One of the changes is that if earlier, when a new notification was sent out, a new one hid the older one, now this does not happen and notifications are accumulated until the user reacts to them or starts the application. The increase in the display of notifications is associated with this - if earlier there could be a maximum of 1 notification from the game in the list, now there can be many of them, and all impressions are counted. Duplicate alerts were also initiated by the developer, but the old system hid such duplicates.
Thus, now for one application per view of the list of notifications, the counter can wind up to 6 notifications at a time (5 mass + 1 simple), before all this collapsed to a maximum of 1 notification.
