Script for project documentation?

There is some kind of small project, something is constantly being completed in it, so you need to keep documentation of changes and additions. Maybe there is some solution in the form of an extremely simple PHP script?
Or all the same to look towards SVN?

It's better to look towards GIT, but that's not the point.
Come up with a beautiful and convenient commit template, and a simple script that will run through git log (svn log) and collect these templates in release notes.
Then it will always be possible to generate separate release notes between any two releases. And it will be more convenient for you.
