Firstly , this is actually a task, not a question. And other people's tasks are not accepted here.
Secondly , in order for this to turn into a question, you need to try to solve it yourself. And when faced with a specific problem, ask about it, for example: You need to do something. I did this <code example>, but I got (a description of what happened) as a result, but I need (an explanation of what is wrong with the result).
And thirdly , let's say I'm in a good mood today, and it's not difficult for me to write a request for you. I click on the screenshot you attached, and everything is so small there that I can't even make out the names of the table fields.
