Try to display captcha in Russian (if the site is for CIS). Hindus and Chinese will no longer be able to enter captcha for half a cent, and I have not seen solving Russian captcha, and if it exists, it is clearly more expensive.
Make your own logic captcha. And change its algorithm (well, or variable names, order) once a month.
if the site is on asp.net, then you can try the NoBot component from AjaxToolkit,
if not, then you can use one of the mechanisms that is used there: if the user clicked on the “send” button faster than N seconds, then the user is a bot, timing N can also save from the Indians, since they do not print something on the form in the case, but stupidly read the captcha and fit in, say, 4 seconds, and a normal user, having typed a message, fits, for example, in 10 seconds.
