Problem with neural network training, how to build mlp?

I can’t figure it out, they sent a manual, but nothing is clear in it. I tried to find it on the net and did not find any results on how to submit a file with training data. The task is to train the developed MLP on the training data, which are attached below. After training, we will be sent test data.
