One password for all, how to prevent password leak? Or vycheslit the one who leaked?

Hello everyone, I am a maintenance engineer for a machine cable company.
We have different devices on which operators work.
There is a type of these devices that weld together several cables, and in order to prevent marriage, these devices measure different parameters by which the quality of the welding is determined.
When these parameters do not fit into the norm, the device gives an error and it is impossible to work further until the maintenance mechanic comes up and sets the password and removes the error.
The problem is that there are many mechanics and only one password can be set, and among these mechanics there are those who give this same password to operators, and since it is the same for everyone, it is impossible to determine who leaked the password.
We issue the password in barcode format.
Is it possible to somehow make a barcode so that when scanning it gives out the same password, but at the same time so that it can be determined who leaked the password?

While writing the question, the following thought came to me:
Put some kind of program in which you can enter with different passwords and then put the same password from it that was mentioned above, well, by itself, it will restrict access only through this program.
Does anyone know if such programs exist?

I will describe how I imagine the second option with the program.
The program monitors the process and quality, and when something does not fit into the specified parameters, it pops up a window with an error and asks for a password, I believe that when this window pops up, a new process starts in the task manager (but did not check).
Further, our program should monitor the launch of the given process, and as soon as it starts, the program immediately displays its window with a password request and until you enter one of the correct passwords, do not give access to other actions.

Address this problem to the one who made the program - so that there might be. many passwords.
And in general - demand to open the source code or at least the API for plugins.
