Need to implement a video conferencing site, what to use?

Conferences should include: chat, video and conversation for 2-3 people, ideally with the ability to track when a user has disconnected from the conference.
The anonymity of the conversation is not important - that's why I tend to use javascript (maybe something else).
What would you recommend to use, are there ready-made solutions or manuals that helped you create something similar?
