The kernel is old, the version of PHP is old... the first thing that comes to mind is to transfer everything to hell to a more recent OS, thoughtfully setting it up along the way.
Or wait - is it some kind of shared hosting, or what? Then you can move your hands for a very long time - and the reason will be in some thread of a voracious neighbor.
