Is there an Android application that makes it as easy as possible to create a form / questionnaire, the data of which will be saved in Excel or CSV format?

Good afternoon, dear Habr users!
On duty, it is often necessary to develop various forms in Excel, which are issued to employees for filling out (on a laptop or in a printed form) while they are on the objects "in the field". Of course, this is not always convenient. You can't always drag a laptop away (especially if you work on a rooftop or a cell tower), transferring data from a paper form to a spreadsheet is also the most enjoyable activity you can name.
Is there any android application that makes it as easy as possible to create a form/questionnaire whose data can be saved in Excel or CSV format?
If there is no such application, then what package / environment would you recommend in order to write such an application yourself.
I will say right away that learning to develop on Android is a secondary goal here.
A long time ago at school I was fond of programming in Basic, VB and Pascal, but this is far in the past. Now I am engaged in cellular communications, mainly engineering the processes of installation of equipment for switching and radio subsystems. Brains have not dried up yet :)
I will be grateful for the answer!
