Is there a sample report on an internal audit to identify changes in the processing and protection of personal data?

Hello!
An inspection from the prosecutor's office came to our organization. They believe, among other things, the protection of personal data. We requested reports on the conduct of internal audits:
- conducting an internal audit to identify changes in the mode of processing and protecting PD;
- organization of analysis and revision of existing threats to the security of personal data, as well as prediction of the emergence of new, still unknown, threats.
These reports must be made annually. We do not have these reports. All other documents on the protection of PD are available (last year they were tested by Roskomnadzor), i.e. the time for such checks has already passed. There is no understanding of what exactly should be in these reports. On the Internet, it is impossible to find examples and samples of reports with just such names. How to make them at all? Ideally, of course, to see examples of how to fill out these reports.
Well, or just maybe someone will tell you - how to get rid of this issue from the prosecutor's office?
