In general, even in Word you can do it.

Any CRM systems in one form or another have the functionality of survey scripts, where it looks like "just a conversation" goes according to the script and at each step the operator fixes the answer or assessment. And this is only a part, because it is followed by analytics in different sections of customer satisfaction rates, operator activity, etc.
