When generating an email template, all styles must be inline. That's it, note. As for the form, it contains specific tags like form, input. Each web email client processes the HTML content of the email in its own way and cuts / leaves the tags it likes. So there is no way to guarantee the work of such a form by 100%, you need to check it at least on popular clients. Offline clients will most likely display the form properly, and setting target="_blank" will most likely work.
