1. The main service (Bitrix24) cannot respond to the /start command from the user.
2. Therefore, you need to connect an additional service that will immediately ask the bot user a question on the /start command.

But it is potentially possible to send messages to users from different sources. True, it will be necessary to establish the exchange of information on users between these two sources. Which in itself is a bit of an odd task.
