This is a stock image of a tombstone . Much more interesting, indeed, is how the advertised site managed to add it.
If I were you, I would carefully study the page code when it appears - perhaps something can be understood from the names of tags, divs, and the like.
Another option, if you have a client paying Google, is to write to support and directly ask "how can I also screw this up?". Paid Google users with a scratch, but usually answers.
I don’t have this link in the results, I didn’t see such a picture, but ... I can assume that this is one of your advertising companies or advertising companies of your partner friends or some other connection, you are authorized in Google / browser (I see this is mobile) and, accordingly, Google this links, personalizes and tags the ad as 'your' ( or in this case your client of which you are the user/admin of the ad account ).
Why do I think so? because all sorts of hamsters, chipmunks and others appear as icons on the top right when you give a Google document to the public, and everyone who watches it is marked like that (if not authorized, it will be like an unidentified chipmunk)
