How to withdraw google adsense income to tks card?

I apologize for the noob question, but nevertheless:
A couple of years ago I threw a Google Adsense code on an ownerless site and during this time 200 bucks dripped there, which I finally decided to withdraw to the card (an alternative withdrawal method between an inconvenient check and rapid), but caught myself at the thought that I don’t understand how to correctly fill in the data for an international transfer (I have never received money from abroad before), and if you fill it out incorrectly, then there is a high probability that Google will not let you change the data and the money will hang to the end, so I'm asking for help!

A big request from those who understand these things, and especially those who have already withdrawn income from edsense to tks cards, suggest the following things:
1) is it possible to receive international transfers if the card is ruble (will there be auto-conversion)?
2) I enter the account number the one that is the Russian bank account (408178...) or the bank's correspondent account?
3) do you need to enter the details that are optional, according to Google, for subsequent enrollment? (I repeat, it is desirable to learn the info from TCS cardholders)
4) is the name of the account holder entered without a patronymic?

4. the name is entered as indicated in the attached instructions (i.e. with a patronymic)
in the column for subsequent enrollment, it is better to indicate the contract number + full name in Latin.
