How to view apache activity?

Я хочу видеть в консоли следующую информацию:
1. IP адрес пользователя
2. URL к которому он обращается
3. PHP скрипты или другие файлы, которые исполняются для этого господина.
Не получается собрать эту информацию воедино. По отдельности вроде могу найти информацию, а вот связать её воедино не получается. Помогите пожалуйста.

For test conditions, Fiddler can be used . Yes Yes. I didn't make a reservation. It is fiddler. And it is for debugging on the server. Just need to poke around a bit.
