How to set up a smart box router from Beeline on MTS?

There is Internet from MTS.
Wi-Fi needs to be set up.
I go under SuperUser.

What should be under - CONNECTION NAME?
What is the IP MODE?
Which account to use? From lx?
And what to enter instead of L2TP Server? What I found somewhere on the Internet. I have no idea if it's working.
