How to recover timecodes in RAW h.264 and AAC without transcoding?

Is there a program that will put the correct DTS / PTS (or what is there instead of them?) in RAW h.264 video and RAW AAC audio *without* re-encoding?
Or better yet, remix TS -> MP4 (h.264 video and AAC RAW audio) with the correct DTS/PTS values.

You need to take the raw stream out of the original container, then generate the timecode and put everything in the new container. At least ffmpeg won't do genpts in the process of multiplexing from one container to another.

I will add that this will only work if you have a constant frame rate and sample rate - it is impossible to guess the timecode with a non-constant one.
