How to properly organize/store the list of features/bugs for a git project?

new features are regularly added, bug fixes, how can I organize a list of what has been done and fixed in the form of a file? in the wiki? in the readme? Maybe there are tools for this?

There are both wiki and issues on Github/Bitbucket/Gitlab.
If you need a separate bug tracker, then there are a lot of them - choose according to your taste.

Everyone decides for himself.
Who writes in the readme, who writes in a separate file, who spins the redmine, who writes on which thread of the git hosting wiki.
In general, this is not the task of the git, but the organization of work on bugs and it is necessary to approach the task from the "management" side.
I have all the repositories on my gitlab server and everything is solved with the help of issue.
