How to monitor SSD?

Recommend a program that will monitor the number of reads and writes from autorun to shutdown.
I have a monitoring need that will document the read and write counts separately for each process each day.
If possible, I would like to have such a program on Windows and Linux

In windows - standard Windows performance counters. (Performance Monitor)
On Linux, iotop, iostat.
