How to model tables that should store user data (in particular, their names)?

Let's say a medical institution needs to store data about their patients. If everything is quite simple with Russian names (we have a first name, last name and patronymic), then what if you also need to store user data that belongs to other nationalities and cultures (for example, Spaniards can have several "surnames" and first names)?

If you want for all cultures at once, then two fields are enough - the full name and the short name. Moreover, both fields are without restrictions on the character set and length.
