How to make the same block for setting the route as on Yandex Maps?

I did not find this type of control in the documentation (namely, a form with 2 fields, as in the screenshot). Is this a unique element for the map or is there a ready-made one somewhere? I don't want to fence the bike if it's already made by someone :)
