How to make one form from the many to one relation in symphony?

Hello.
The task seems to be simple, and I read the documentation, but I can not understand one thing.
There is a user entity and a company entity.
Company has multiple Users (one to many).
I want to create an empty form with two fields:
user_name (belongs to the User class)
company_name (belongs to the Company class)
How to use formBuilder for such a form?
