How to make a survey page in wordpress?
It is necessary to make a page so that people fill in the required fields, attach a photo, and all this information goes to my site.
I tried to do a similar form through CF7, but it did not work to send the image. For some reason the email is not coming.
Are there other useful plugins?

Specifically, this error is simply about the fact that nothing happened to collect. The main errors are higher in the log. Here they should be brought. +add info about how you build (gredle, minification, target architectures, IL2CPP/Mono) and if there are any plugins.

Perhaps you are inserting the shortcode of the attached file into the body of the email. There is a special field for it just below.
