The site has a "Request a callback" form.
Name, phone number and email are requested.

What needs to be done. other than publishing a policy?
Has anyone come across? What will be the next step of Roskomnadzor?
If, for example, you completely remove the form from the site, will the problem be closed on this?

So take it and publish on the site the procedure for processing user data.
When sending data, the user must agree to your terms, for example, by checking the box.
Take the text of the agreement on the processing of personal data on the Internet. Make the checkbox with confirmation by default checked.

Ru-Center ( https://nic.ru ) has a hosting service in compliance with Federal Law 152.
This is the only cheap option without further sanctions.
