How to log file uploads at apache level?

It is required at the web server level to log all file uploads to php for site security audit. In the logs, you need to see the PHP script with which the file was uploaded to the site. Are there ready-made solutions / mods to complete the task?
Debian server running Apache 2.2.16 + Nginx 0.7.67 + PHP 5.3.3-7 + squeeze15
Thank you!
