How to insert a photo into a frame and save it all in a graphic format?

Actually, I can insert a photo into a frame, like placing a div with a frame over another with a photo, but how do normal people save, for example, spreads for photo books (from some online editor) in pdf, tiff, jpg and even with normal resolution?

I agree with Ivan, you need Canvas. If plain vanilla canvas is too tight, you can use some library like fabric.js .
