How to edit PDF Questionnaires with Python?

There is a form in PDF format (example attached). This questionnaire needs to be filled with certain data, ticks through Python. How could this be implemented? Is there any library for this?

You create this primitive layout in HTML.
Complete the text and attributes with Python.
Convert to PDF.
And no rectal tonsillectomy.
