How to create a form for adding records?

I want to maintain something like a small database of entries in Google Spreadsheets. Is it possible to somehow create a form that will add a new row to the end of the table? Just to avoid manually creating newlines. Plus, there are some drop-down lists, checkboxes, etc.

Make google forms (there are check boxes and lists), bind the current table in the settings. When filling out the form, the data will fall into the table.
