How to create a comment in pdf?

I need to find and select a word or select text by position (like Range in word) and create a comment in this place.

What libraries can you recommend?
If there is a small vbscript example, it will be even better.
Before that, I worked with pdf through pdftoolkit, but either I'm looking badly, or there is no work with comments.
