How do I combine a win and a combination into one hashtag?

On the screen there is "#P2_ Pair" I need to combine them into one hashtag. There is a $Combination variable from which all the information comes. But it displays for example: "Player 1 won with a combination of Two pair!". Therefore, I replaced "Player 1 won with a combination" via str_replace with "W1", and then only the combination goes through $Combination. And I need to connect the victory, as well as the combination. I repeat once again that the combination comes from $Combination
