No way, you can encrypt everything up to your gateway, and then everything that goes to the PSTN is open. GSM traffic is also hacked ( habrahabr.ru/post/79631 )
The only more or less stable way is client <=> SRTP - your server is SRTP <=> client , where there is a 2nd side there will always be "rotten" :)

In this scenario, at best, you can make an anonymizer, that is, the receiving party will not know who is calling and from where. Everything that is connected with correspondence or conversation on a standard GSM phone - without additional funds on both sides, is indefensible in principle. About changing the number on the fly (during a conversation, or what?) I did not understand.
