How to auto-add code?

Hello. I have a gallery. Made it just for fun. Now I want other users to be able to add photos there. I would like to make it so that when you add a photo, text to the fields and click on the button (Example in the photo (Design VK for example)) , a code is automatically created in a certain place, and text is inserted into this code in a certain place, and in certain place photograph.
I'm new so don't judge too harshly :)

What exactly is the problem?
The picture is selected, the text is written. Next:
- upload the file (picture) to your server
- remember in the database or wherever you want the path to this picture and the signature (text)
- generate the html markup (template) to display the picture with the caption
- add a new picture to the gallery
